My instant response to this question was Slayer's Reign in Blood. The whole album's brilliant but it all builds up perfectly to the final couple of tracks.
Two of the early entries here Down in the Sewer and Won't get fooled again probably fight it out for my best finishing tracks to an album, but in the last three department I offer:

Couldn’t be arsed to read all the other entries. I know it’s not listed on the vinyl (as I’ve got it) but as far as I’m concerned it’s a real track. Seem to remember reading somewhere HM is one of the shortest songs ever recorded. I like it anyway...
